Exclusions & Limitations.
This Accidental Death Plan provides $50,000 of coverage.
Individual and spouse coverage.
You can choose individual coverage for just yourself, or choose to cover your spouse too.
You can’t be turned down.
Your acceptance is guaranteed regardless of your health or occupation.
Exclusions.
For group coverage in the states of AZ, CA, CT, DC, KY, MA, MO, NE, NJ, OH, PA, RI and TN, the exclusions are as follows (Certificate SLC-AD75-0902):
The Certificate does not cover death caused by or contributed to by any of the following:
| 1. | Intentionally self-inflicted injury, suicide or any attempt at suicide while sane or insane (in Missouri, while sane) |
| 2. | The Insured having a blood alcohol level of .10% (by weight or volume) or higher |
| 3. | Taking or using any narcotic, barbiturate or any other drug, unless taken or used as prescribed by a physician |
| 4. | Taking of alcohol in combination with any drug, medication or sedative |
| 5. | Any sickness or its medical or surgical treatment (except bacterial infections which result from an Injury or accidental ingestion of contaminated substances) or Mental Disease or Disorder |
| 6. | Voluntary gas inhalation or poison voluntarily taken, administered or inhaled |
| 7. | Air travel as a pilot or crew member |
| 8. | Committing or attempting to commit any illegal activity |
| 9. | Participating in any race or speed contest involving motor vehicles of any type |
| 10. | Any active participation in a riot, insurrection or war, either declared or undeclared |
For individual coverage in the states of CO, DE, ID, IL, IN, IA, KS, ME, MI, MS, NV, OK, SC, SD, TX, VT, WI and WY the exclusions are as follows. Some of these states have exceptions to these exclusions. See below for the exceptions.(Policy Series SL-AD75-1002)
We do not pay any benefits if any of the following contributed to or caused your injury:
| a. | Intentionally self-inflicted Injury, suicide, or any attempt at suicide while sane or insane |
| b. | Any sickness or its medical or surgical treatment, including diagnosis (except bacterial infections which result from an Injury or accidental ingestion of contaminated substances), or Mental Disease or Disorder |
| c. | Having a blood alcohol level of .10% (by weight or volume) or higher |
| d. | Any active participation in a riot, insurrection or war, either declared or undeclared |
| e. | Committing or attempting to commit any illegal activity |
| f. | Air travel as a pilot or crew member |
| g. | Participating in a race or speed contest involving motor vehicles of any type |
| h. | Voluntary gas inhalation or poison voluntarily taken, administered or inhaled |
| i. | Taking of alcohol in combination with any drug, medication or sedative |
| j. | Taking or using any narcotic, barbiturate or any other drug, unless taken or used as prescribed by a Physician |
Important State Exceptions: If this Policy is issued in the following states, these exclusions replace the corresponding exclusion shown above. In Idaho: exclusion, (c),(e),(g),(h) and (j) are deleted. Exclusion (b) is changed to "any sickness (except bacterial infections which result from an injury or accidental ingestion of contaminated substances), or Mental Disease or Disorder." Exclusion (i) is changed to "alcoholism or drug addiction." In Illinois, Iowa and Michigan: exclusion (g) is deleted. In Michigan, exclusion (j) is changed to "the Insured's voluntary use of any illegal drug or controlled substance as defined by federal or state laws unless used as prescribed by the Insured's physician." In Oklahoma, exclusion (d) is changed to: "participation in a war or act or war declared or undeclared while serving in the military service or any auxiliary unit attached thereto". In South Carolina, exclusions (c) and (g) are deleted. Exclusion (j) is changed to: "being under the influence of alcohol or any narcotic unless taken on the advice of a physician;" In South Dakota: exclusions (c) and (i) are deleted. Exclusion (j) is changed to "being under the influence of alcohol or any drug while committing a felony." In Texas: exclusion (c) is changed to "treatment for the disease alcoholism." In Vermont, exclusions (c), (g), (h), (i) and (j) are deleted. Exclusion (e) is changed to: "commission of or attempting to commit a felony; or being engaged in an illegal occupation;" In Wisconsin: exclusion (c) is changed to "the Insured being intoxicated unless administered on the advice of a Physician."

Important Note: We may not be able to issue coverage if you are replacing existing coverage. You can be covered under more than one Stonebridge Life policy/certificate providing the same or similar coverage subject to our right to limit the total face amount of coverage to $500,000. (In South Carolina, this provision applies only to total insurance provided by individual policies.)

Important Note: You can be covered under more than one Stonebridge Life policy/certificate providing the same or similar coverage subject to our right to limit the total face amount of coverage to $350,000.
